The Care Coordinator conducts behavioral health and substance abuse assessments and treatment referrals.

Must hold a Master's in counseling, social work or related field. LPC, LCADC or LCSW with co-occurring experience strongly preferred or documented evidence that a license will be obtained in the next year. Must have a minimum of two years co-occurring disorder experience and extensive applied knowledge of The ASAM Criteria, 3rd Ed. and the DSM 5.

A Bachelor’s Degree in counseling, social work, or related field will be considered for positions located in Essex and Camden Counties only. CADC is required. Must have a minimum of five years co-occurring disorder experience and extensive applied knowledge of the DSM 5 and The ASAM Criteria, 3rd Ed.
