However, extra time may be needed if materials are missing or if questions arise during the review process.
Copies of certificates may be submitted with the recertification application.
100% of the requirement recertification hours may be taken by approved distance learning.
$15 per month (or partial month) late fee for late applications. Please include the late fee when submitting your application.
After two years (24 months) of inactivity certifications will be terminated and will require a new application that includes new training and education requirements to be met.
